Medication errors are one of the leading causes of preventable hospitalisation in older adults. With many seniors managing five or more medications daily, the risk of missed doses, double doses, or dangerous interactions is very real. As a caregiver, helping your loved one manage their medications safely is one of the most important things you can do.
Create a Medication List
Start by creating a complete, up-to-date list of every medication your loved one takes — including prescription drugs, over-the-counter medications, vitamins, and supplements. Include the dose, frequency, and what each medication is for. Keep a copy at home and bring one to every medical appointment.
Use a Pill Organiser
A weekly pill organiser is one of the simplest and most effective tools for preventing missed or double doses. Fill it at the start of each week and check it daily. For loved ones who take medications at multiple times of day, look for organisers with AM/PM compartments or separate compartments for morning, noon, evening, and bedtime.
Consider an Automatic Pill Dispenser
For loved ones who live alone or have memory challenges, an automatic pill dispenser can be life-changing. These devices are pre-loaded with medications and dispense the correct dose at the correct time, often with an audible alarm. Some models can even send alerts to caregivers if a dose is missed.
Set Reminders
Simple phone alarms or reminder apps can be surprisingly effective for seniors who are comfortable with technology. Set an alarm for each medication time and label it clearly (e.g. "Morning tablets with breakfast"). For those less comfortable with smartphones, a basic alarm clock or a note on the fridge can work just as well.
Review Medications Regularly
Medications and dosages change over time. Schedule a medication review with your loved one's GP or pharmacist at least once a year — or whenever a new medication is added. Pharmacists are an underutilised resource and can identify potential interactions or simplify complex regimens.
Store Medications Safely
Store all medications in a cool, dry place away from direct sunlight. Avoid storing them in the bathroom, where humidity can affect their potency. Keep medications out of reach of grandchildren and dispose of expired medications safely through a pharmacy take-back programme.
